I acknowledge that I have been given the full opportunity to ask any and all questions about obtaining a tattoo and that all of my questions have been answered to my full satisfaction.

If I have diabetes, epilepsy, hepatitis, hemophilia, HIV/AIDS, jaundice, bloodborne pathogens, heart conditions, take blood thinners, or have any medical condition that may affect the tattooing process or the health of the artist and associates, I have disclosed this. I am not pregnant or nursing and am not under the influence of alcohol or drugs.

I do not have medical or skin conditions — including acne, scarring (Keloid), eczema, psoriasis, rash, freckles, moles, lesions, or sunburn — in the area to be tattooed that may interfere with tattooing or the healing process.

I acknowledge it is not reasonably possible for Ornamentalika to determine whether I might have an allergic reaction to the pigments or processes used, and I agree to accept the risk that such a reaction is possible.

I acknowledge that infection is always possible as a result of obtaining a tattoo and I assume that risk. I have received aftercare instructions and agree to follow them. Any touch-up work needed due to my own negligence will be done at my own expense.

I understand that variations in color and design may exist between the tattoo as selected and as ultimately applied. Dark skin tones may cause colors to appear less bright. Poor aftercare may also result in variations.

I understand that skin treatments such as laser hair removal, plastic surgery, tanning, or other skin-altering procedures may result in adverse changes to my tattoo.

I understand and acknowledge that the tattoo procedure will cause a permanent change to my appearance and can only possibly be removed through surgical procedure, which may result in permanent scarring and/or disfigurement. No representation has been made about my ability to later remove or alter my tattoo.

I understand that the tattoo normally takes 2 weeks or longer to heal. I do not have a physical, mental, or medical impairment that might affect my well-being as a result of having a tattoo done at this time.

I acknowledge that I am over 18 years of age and have truthfully represented that obtaining this tattoo is my choice alone, made of my own free will.

I will be tattooed using appropriate instruments and sterilizations as determined by Ornamentalika, and I consent to the application of the tattoo and any actions deemed reasonably necessary to perform the procedure.

I hereby release all rights to any photographs/videos taken of the tattoo and of me in connection with this tattoo, give consent in advance to their reproduction in print or electronic form, and understand that Ornamentalika reserves all rights to use these photographs.

By signing this form I agree to waive all legal rights to file suit or claim against Ornamentalika, its employees, associates, or affiliates. I acknowledge that I am of sound body and mind and am acting on my own cognizance in consenting to this procedure.

I hereby release and forever discharge and hold harmless Ornamentalika, the tattooist, and all affiliates, owners, managers, employees, representatives, and agents of Ornamentalika from any and all claims, damages, or legal actions arising from or connected with my tattoo and/or the tattooing procedure, to the fullest extent allowed by law.
